This demo policy explains the intended privacy posture for the ClearPath ADHD website rebuild. It is not a final legal document.
Introduction
ClearPath ADHD is presented here as a demonstration healthcare-adjacent website. The production site should use a privacy policy reviewed by qualified counsel and aligned with the actual clinic workflows, vendors, forms, analytics, and clinical systems.
Data collected through forms
Website forms may collect contact details such as name, email address, phone number, state, service interest, and a short message. The ADHD self-check may collect quiz answers, score range, and optional email address if a visitor asks to receive results.
Public website forms should not be used for urgent issues, detailed medical history, diagnosis details, medication instructions, or protected health information beyond what the form clearly requests.
How information is used
Information submitted through the demo site is intended to help respond to inquiries, route scheduling requests, provide requested quiz-result follow-up, improve website reliability, and understand broad site usage.
Third-party services
A production version may use third-party services for scheduling, analytics, hosting, email delivery, and form processing. Examples may include a scheduling platform for booking calls and privacy-conscious analytics for aggregate website measurement.
Retention
Production retention periods should be defined by clinic policy, applicable law, vendor contracts, and clinical record requirements. Demo form submissions should be treated as temporary test data unless production systems are configured and reviewed.
